Ústav zdravotnických informací a statistiky ČR („ÚZIS ČR“ nebo „Ústav“) je organizační složkou státu, jejímž zřizovatelem je Ministerstvo zdravotnictví. Podle zákona č. 372/2011 Sb., o zdravotních službách a podmínkách jejich poskytování (zákon o zdravotních službách), je ÚZIS ČR správcem Národního zdravotnického informačního systému (NZIS).
